Cannot add Image to more than one Image Field when adding Custom Field Set to a Product

Bug reported by Kalyani:
When adding a Custom Field Set to a Product, she is able to add one Image to one Image field, but after updating the Custom Field Set to have 14 Image fields instead of just one, she is now unable to update.
