Files - Adding Files in Rich Text Editors show as broken images

When adding a file from File Manager into a Rich Text Editor field, this shows as a broken image and does not output correctly.

    If a file is entered from the file manager in a RTE then it will now output within an a tag
    Images will continue to render within img tags
